Resolved Scythe of the Wretched and Unscythe, Killer of Kings trigger on every death

Scythe of the Wretched (Mirrodin) and Unscythe, Killer of Kings (Alara reborn) trigger on every death not just when a creature that was dealt damage by equipped creature and causes the to work in a completely broken manner from how they should.

These are very common to see in commander in paper but on MTGO they are un-useable due to the nature of the bug.

I submitted this request (13537) but I want to put it here aswell so people can upvote it.
 
its been a while since ive seen as none of my decks currently use scythe ti but this also happened to me when using scythe

I ran a Tetsuo Imperial Champion deck the other day and confirm both the triggers when they should not triggers and the swamp tokens

When tested Scythe of the Wretched will incorrectly trigger off of a creature dying in combat when it isn't equipped to a creature. Once Scythe of the Wretched is equipped to a creature it appears to function as expected, and no longer reanimates creatures not dealt lethal damage by the equipped creature.

My testing with Unscythe, Killer of Kings appears to mirror my results with Scythe of the Wretched where it will incorrectly trigger when not equipped to a creature, but appears to function as expected once it is equipped to a creature.

In my Game Scythe reanimated creatures even though the equipped creature dealt no damage to that creature. It wasn't even equipped. Every creature that died just got reanimated by the Scythe.
 
Scythe of the Wretched reads "Whenever a creature dealt damage by equipped creature this turn dies, return that card to the battlefield under your control. Attach Scythe of the Wretched to that creature."
How it is functioning in game is "Whenever a creature dealt damage by a creature this turn dies, return that card to the battlefield under your control. Attach Scythe of the Wretched to that creature."
This is not correct, and makes the card significantly stronger.
It isn't a may effect so I can't even choose to not use the exploit other than by not using the card.

In this game, my Liliana. Heretical Healer, was destroyed in combat by an opponent's Maelstrom Wanderer while I had a scythe in play (not attached to any creature) and controlled no other non-land permanents.
I cast an Undying Malice targeting my Liliana, to get the creature back after combat, however, when she was destroyed, Scythe's ability brought Liliana back to play (untapped and with no counter, so it wasn't my mistake confusing Undying Malice) and attached itself to Liliana.
