Could Not Reproduce Olivia, Opulent Outlaw failed to trigger from combat damage

Was recently playing a game and had a combat step where Olivia, Opulent Outlaw failed to trigger from combat damage when the game log clearly shows that combat went though since I took the Monarch. Olivia functioned as expected multiple times earlier this game, and later this game after I recast her she resumed functioning as expected. The only thing that happened to that specific instance of Olivia that isn't very common is that an opponent phased her out with an Out of Time, but that shouldn't have had any effect on her after she phased back in.

I haven't been able to reproduce this in testing, but I'll update in the future if happens again. Hopefully it was just a weird one of glitch, and not a bug that only triggers under certain conditions.
