This happened in a Cube30-draft.
My opponent had Wildborn Preserver in play, and I had Teferi, Master of Time.
So, they had a creature come into play (a Saga that transformed), and I responded to the trigger from Preserver with Teferi’s phase out ability.
Opponent put two mana into the ability from Preserver, and it, phased out as it was, got two +1/+1 counters.
A permanent that is phased out is supposed to be treated as though it does not exist, and you clearly can’t put a counter on something that doesn’t exist.
