I have seen this happen multiple times before but just forgot to report it.
mothman was triggered by an attack by Six which milled 3 nonlands.
as per gatherer "The cards are milled all at once, which means abilities that trigger "whenever one or more nonland cards are milled" will trigger exactly once as long as at least one nonland card was milled."
however instead of 1 trigger with x being 3 to put 1 counter on up to x creatures it was 3 individual triggers which means that you can put more than one +1/+1 counter on one creature.
this means that mothman can make creatures far larger than they should be off one single mill event
