Confirmed O-Kagachi, Vengeful Kami seems to be bugged... again

Hello, I wanted to tell you that there are some cards that do not work properly or that block the game.
'O-Kagashi Vengeful Kami' does not trigger when a player attacked the owner the previous turn and the owner attacks him in return.
'Lavabrink Floodgates' remains blocked once its activation when it should sacrifice itself, it does its damage trigger again and again without stopping.
'Plaguecrafter' does not discard an opponent when it does not make the sacrifice.

Please provide game ID, and screenshots if possible, with bug reports. Also, pleas make separate posts for each bug.

Plaguecrafter is a known bug that I hope gets fixed soon because it does see a lot of play in regular commander. Doubly so since it is also part of the free Braids deck given out to new accounts.

Orginal post here made me curious because both older reports on O-Kagachi, Vengeful Kami here and here have been marked as resolved. Upon testing, O-Kagachi, Vengeful Kami does appear to be bugged again with it's ability not triggering as expected.

I will make another post for each cards but i can't take screenshot for the bug with O-Kagashi Vengeful Kami Because the trigger doesn't.
