This bug is easily reproducible as it happens to me every time I try.
To reproduce:
Step 1: Opponent casts any spell.
Step 2: When you receive priority to respond, Tap mana sources to generate mana for Kozilek's Command.
Step 3: Click on Kozilek's Command.
What should happen: A dialog box should pop up allowing you to choose which two modes you want, allowing you to respond to whatever it is your opponent is casting.
What is actually happening: The mana remains in your pool, priority passes back to opponent, and the spell they cast resolves. You are never actually given the opportunity to cast Kozilek's Command.
